Overview

ACORN - Assessment of Clinical Oral Risk and Needs This guide is to support the detailed guidance issued by Welsh Government on the 22nd May 2020.”

The NHS Wales Payor has a revised tick box on Payor File: “Auto chart NHS Risk Service on COT creation”. It is on by default and will automatically add “NHS Risk Assessment” service to planned UDA COTs.

NHS Risk Assessment service (as before) will be automatically to NHS UDA COT:

Ticking the NHS Risk Assessment Service will launch ACORN questionnaire with four tabs - Example:

The three ‘Yes, No or Unset' buttons will send ‘Amber’ or ‘Green’ status in the claim XML.

Warnings will be displayed to user if there is missing data when trying to tick NHS Risk Assessment complete.

Medical History

If your practice uses Fixed Medical History ‘Default’ or ‘Advanced’, some options will pre populate. It will answer Amber on Behaviours and Protective Factors tab e.g.

Using the Advanced Fixed Medical History form, we tick ‘Lots of fizzy/acidic drinks’ for a patient:

On an NHS course of treatment, mark the NHS Risk Assessment service as complete. NOTE: Behaviours and Protective Factors tab pre populates Yes answer (Amber) for “Consumes… fizzy drinks etc…” question e.g. below (user can change the answer to No if desired):

Next, go to the Clinical Findings tab. When you tick ‘Green’ under the Tooth Decay (for dentate only) section, the Overall Tooth Decay Score will tick itself Amber (because Behaviours tab has an Amber) e.g.

Workflow for Patients Under 12yrs old.

The “Patient is under 12” tick box will automatically be ticked when completing ACORN questionnaires for patients under 12yrs old e.g.

The ‘Periodontal Health Specific Risks Age 12+ Only' section is NOT displayed for under 12 e.g.

View Risk Assessment - completed, TCd claims

Read only answers ticked (or text entered) will be visible for completed (or TCd) claims when choosing 'View Risk Assessment” from Tooth History or Edit Chart Item. Chosen Yes, No or Unset answer buttons will have a coloured edge. See examples below.
