For practices in Wales you will be required to fill out an https://soeidental.atlassian.net/wiki/spaces/PHS/pages/836370538 as part of an NHS Course of Treatment. Warnings will be displayed to user if there is missing data required to be sent with the FP17W claim when trying to tick NHS Risk Assessment complete.
