An online COVID screening form [NZ]

In 2019 EXACT added a new feature called Patient Portal to the group of features known as Patient Experience.  

The Patient Experience features help your team provide contactless customer service. Patient Experience features include Online Booking, Clinipad and Patient Portal.  

The Patient Portal is available so your patients can complete relevant forms at home on any device with an internet connection (i.e. a smartphone, laptop, computer or tablet).

About Patient Portal

As part of the Patient Portal installation, our team will add Smart Links to the SMS and Email message sent as Automated Appointment Reminders.   The Smart Link will check the patient file before sending an appointment reminder.

If any of the forms below are missing or out of date on the patient file; the reminder message will include a link to the Patient Portal.   Here are the forms which seamlessly update the Patient file in EXACT once completed on the Patient Portal.

  • The Patient Details form (as per the details tab of the Patient file)

  • The Medical History form

  • COVID-19 Screening Questionnaire

  • Contact Consent

  • Oral Health Survey (optional)

  • Terms and Conditions (optional)

Please Note: The Patient Portal forms are not customisable. The Patient Portal forms have been developed to ensure patient information is saved to relevant fields and tabs in the patient file in EXACT.
