What Forms Are Available?

Practices must use the 'Advanced' Medical History form, if you are using Default and need to switch the data will be transferred over so patients don't need to complete a new form.

If you are using a form type that is not Default or Advanced prior to Patient Experience, patients will need to complete a new blank form.

Patients can select only 'YES' answers if they desire.

**Note: currently on patient portal this form is not customisable, it will ask all questions regardless of if you haven’t unchecked some within Exact.

These must be accepted by the patients in order for them to access the portal.
